Dear Patient,

We hope this finds you and your family in good health. Our community has been through a lot over the past few months, and all of us are looking forward to resuming our normal habits and routines. While many things have changed, one thing has remained the same: our commitment to your safety.

Infection control has always been a top priority for our practice, as you have witnessed during your visits to our office. Our infection control processes ensure that when you receive care, it is both safe and comfortable.

Our office follows infection control recommendations made by:

American Medical Association and CDC and the Occupational Safety and Health Administration (OSHA).

We closely monitor these agencies so we are up-to-date on any new rulings or guidance that may be issued.

Due to the pandemic we are sorry to inform you that we will no longer accept walk-in patients.

Before your visit

  • 1. Our office will contact you before your appointment with a screening questionnaire and any information that is needed to complete your appointment. If we do not receive your completed questionnaire or information required at least two hours prior to your appointment or a day before, your appointment will automatically be cancelled. Please be sure that we have your most updated information on file and be ready to answer your phone. There will be no exceptions.
  • 2. Check your temperature at home before you come for your appointment. Your temperature will be checked when you arrive at our office. If you have a fever, or develop any ill symptoms your appointment will be rescheduled. This is for your safety and those around you.

On the day of your appointment

  • 1. Patients are required to wear face covering (mask) at all times while in the office, no exceptions will be made. We require that you bring your own mask. Masks will not be provided at our facility.
  • 2. PLEASE do not wear gloves. You will be asked to dispense gloves if worn. This helps eliminate the spread of germs in our facility. We will provide hand sanitizer through out your visit and may ask you to wash your hands with warm water and soap.
  • 3. Please knock on the door before entering our facility and wait until we call you to come inside the office.
  • 4. Only patients are allowed inside the office. Our waiting area is closed. Anyone who is accompanying a patient should wait outside of the building.
  • 5. Appointments will be managed to allow for social distancing between patients. That might mean that you’re offered fewer options for scheduling your appointment. We ask that you place an appointment with an advance time. This will allow courtesy to all patients that are in need of assistance.

We will do our best to allow greater time between patients to reduce waiting times for you, as well as to reduce the number of patients in the reception area at any one time.

We ask all patient to comply with our office guidelines as this will provide a safer atmosphere.
